Point-of-care INR coagulometers for self-management of oral anticoagulation: primary care diagnostic technology update.

چکیده
British Journal of General Practice, November 2012 e798 ADVANTAGES OVER EXISTING TECHNOLOGY Devices to measure international normalised ratio (INR) are intended for both professional use and patient self-testing to monitor oral anticoagulant therapy. INR is a standardised measurement of the prothrombin time, which is the time it takes blood to clot after addition of tissue factor. Point-of-care (POC) INR testing within primary care settings eliminates delays in waiting for the result of prothrombin time measurement to be processed by the hospital laboratory, and the subsequent delay in adjustment of anticoagulation dosing. It also avoids the need to attend hospital anticoagulation clinics.

Factors determining patients’ intentions to use point-of-care testing medical devices for self-monitoring: the case of international normalized ratio self-testing
PURPOSE To identify factors that determine patients' intentions to use point-of-care medical devices, ie, portable coagulometer devices for self-testing of the international normalized ratio (INR) required for ongoing monitoring of blood-coagulation intensity among patients on long-term oral anticoagulation therapy with vitamin K antagonists, eg, warfarin. METHODS A cross-sectional study that...
متن کاملIs point-of-care accurate for indicating thrombolysis in anticoagulated patients on oral anticoagulation treatments?
UNLABELLED The use of oral anticoagulation treatment (OAT) in patients with an international normalized ratio (INR) higher than 1.7 is a contraindication to thrombolysis in acute ischemic stroke. The aim of the present study is to compare the use of point-of-care (POC) coagulometers to the standard coagulation analysis (SCA) procedure of the INR as a decision-making test for use with patients t...
متن کاملSMART: Self-Management of Anticoagulation, a Randomised Trial [ISRCTN19313375]
BACKGROUND Oral anticoagulation monitoring has traditionally taken place in secondary care because of the need for a laboratory blood test, the international normalised ratio (INR). The development of reliable near patient testing (NPT) systems for INR estimation has facilitated devolution of testing to primary care. Patient self-management is a logical progression from the primary care model. ...
متن کاملA randomised controlled trial of patient self management of oral anticoagulation treatment compared with primary care management.
BACKGROUND The increase in numbers of patients receiving warfarin treatment has led to the development of alternative models of service delivery for oral anticoagulant monitoring. Patient self management for oral anticoagulation is a model new to the UK. This randomised trial was the first to compare routine primary care management of oral anticoagulation with patient self management. AIM To ...
متن کاملSelf-testing and self-management of oral anticoagulation therapy in children.
Children and adolescents on oral anticoagulation therapy (OAT) present special challenges in terms of rapid fluctuations in International Normalised Ratio (INR) values, interruption in daily life due to frequent hospital/doctor visits, and difficulties and pain in the performance of venepuncture. Optimised management of OAT improves the quality of treatment, potentially accomplished by new meth...
